Pet HMO Terms of Agreement

Payment in full is due at the time of enrollment.

County Line Veterinary Hospital agrees to provide discounts on veterinary services as outlined below.

Client acknowledges that the discount plan is being offered and honored by County Line Veterinary Hospital and that he/she will not receive discounts on services other than at County Line Veterinary Hospital or Heritage Veterinary Hospital in Delran, New Jersey.

Upon the first examination after or at enrollment, the veterinarian will determine any pre-existing conditions with the pet. At this point, any conditions found are excluded from the discount plan until the pet has not shown any direct or indirect symptoms of the condition for six months. After that time, if the condition recurs, it will be included in the discount plan.

Exclusions: pet foods (all types including prescription foods), non-medical purchases, boarding and grooming services, crematory services and pre-existing conditions.

Clients agree to follow the routine, preventative vaccination schedule as set by his/her veterinarian. If conditions develop as a result of not following the schedule, discounts will not be given on treatment of those conditions.

DISCOUNT PLAN:

25% discount on therapeutic and preventative medications, laboratory tests and X-rays.

50% discount on examinations, consultations, anesthesia, vaccinations, hospitalization, and dental, surgical and clinical procedures.

Client understands that this is not any form of insurance and that he/she will not be reimbursed, balance billed, make co-payments, meet deductibles or receive discounts for services provided to a pet that are listed as exclusions. This agreement is not transferable among pets nor to any other individual or entity.

Client shall have a three day right of recision (to void contract) at which time, normal fees will be charged. After that time, the policy will be in force for one year, and no refunds of premiums will be issued under any circumstances. In addition, the policy will not be prorated during the contract term. If the policy lapses, and the client wishes to renew it, the effective date will continue from the last day the policy was in force, or new pre-existing conditions may apply.
